When I bought my 5000 mid last year from Sydney Detector Den they did it for me, they then sent on the 5000 to me and I got the pretty gold key ring from Minelab a couple weeks later.

G'day Dave,

The guys at Coiltek filled in ours when we bought them we did fill in the survey sheet which they put in with the warranty and then they sent that off to Minelab.
We did transfer the 2300 warranty over though and had to do that via Email to Sher at Minelab.
Hope this helps.

Cheers.

Mike.
